Invite people from different tasks and working hours. Participation is more than sending out a form. Share what you learned, what you decided and what remains.
These are questions for a workplace walk-round, not a test of employees’ health. At small workplaces, even an unnamed answer can reveal who answered. Avoid detailed free-text fields about private matters.
A short shared introduction
Show the arrangements. Explain who refills, how faults are reported and where to get confidential help. Do it at the start and whenever things change. Let managers practise an imagined conversation and a short note.
