Find what gets stuck.
Look at an actual workplace and shift together with employees or their safety representative. Consider physical conditions, pace, control over breaks and support. This is part of your overall risk assessment.

Note the condition, existing measures, assessed risk, decision, responsibility and deadline. Prioritise by severity and likelihood in your work. Do not postpone an obviously necessary fix because the form is not finished.
The examples are not a finished risk assessment. For safety-critical tasks, chemicals, heat or changed protective equipment, the usual professional health and safety assessment must be part of it.
